In consideration for Disability:IN's consideration of [ORGANIZATION]'s application ("Application") to sign onto the Disability:IN Procure Access Statement ("Statement"), [ORGANIZATION] releases from all liability, and waives it right to sue, Disability:IN, its employees, officers, volunteers and agents (collectively "Disability:IN") from any and all claims which may arise from the application process and its outcomes or activities incidental thereto. If [ORGANIZATION]'s application is accepted, it authorizes Disability:IN to add the [ORGANIZATION] name and logo to the Procure Access Statement on the Disability:IN website and Disability:IN social media.

Disability:IN reserves the right to reject, review, and potentially rescind the participation of any Organization applying to sign onto or that has signed onto the Procure Access Statement that displays conduct contrary to or not in accordance with the eligibility requirements above.
